  • Organize and facilitate care conference with appropriate team members.
  • Connect clients with professional services such as doctors, dentists, mental health professionals etc.
  • Maintain consistent communication with client(s) and/or family members. 
  • Create and update treatment/care plan as needed.
  • Coordinate services such as in-home care, adult day care etc.
  • Assist with management of medications such as reordering and ensuring their records are up to date.
  • Facilitate medication review with doctor or pharmacist.
  • Assist with monitoring patient portal and ensuring that appointments are not missed. 
  • Accompany to appointments.
  • Take detailed notes and distribute to appropriate family and professionals.
  • Advocate for client(s) needs and wishes.
  • Recognize signs and symptoms of underlying illness and follow-up with appropriate action.
  • Assist with Advanced Health Directives
  • Regularly observe and note any changes in status or condition and follow through with appropriate health professionals to ensure client maintains best possible health and wellbeing. 
  • Report any suspected abuse or neglect, including self-neglect, to Adult Protective Services.
  • Collaborate with hospital and rehabilitation discharge planner(s) to ensure a smooth transition home.
  • Assistance to ensure bills are being paid on time.
  • Help when clearing home after loved one passes.
  • Support during relocation from home to Assited Living when living at home is no longer a viable option.
